Function to generate the generic (input and invariance) linear constraints for pwar (PWA rectangular) type approximations, for use with lpSolver.

Syntax
[Ac, Bc] = pwarConstraintsLpsolver(approxInput, M, dyn)
Inputs
- approxInput - input structure, see pwarLPApproxLpsolver().
- M - nv*nx matrix with vertices
- dyn - active dynamics in current region
Outputs
- Ac, Bc - constraint parameters of the type Ac*[K,k].' <= Bc.
